HonorVet Technologies is seeking a Social Work Licensed Clinical Social Worker for a travel job in Newberg, Oregon.
Job Description & Requirements Title: Licensed Clinical Social Worker (LCSW)
Location: Newberg, OR
Duration:26 weeks
Shift: Days – 5x8s (09:00–17:30)
Block schedule required
Weekends required (every other)
Job Responsibilities:
Conduct comprehensive psychosocial assessments and evaluate patient needs.
Develop and update individualized plans of care.
Coordinate discharge planning, including placement, home services, DME needs, and community referrals.
Collaborate with interdisciplinary teams to support patient care and safe transitions.
Educate patients and caregivers regarding discharge plans, available services, and community resources.
Address high-risk patient needs, including vulnerability, safety concerns, and complex psychosocial issues.
Document all assessments, interventions, progress notes, and discharge planning activities accurately and according to standards.
Participate in case conferences and care coordination meetings.
Provide counseling, crisis support, and intervention as needed.
Maintain knowledge of major disease processes and their psychosocial impact.
Utilize Epic for documentation and care coordination.
Float to additional units or nearby affiliated facilities as needed (up to one day per week).
Manage patient caseload with an expected ratio of approximately 1:20.
Required Skills & Qualifications:
Active LCSW license (Oregon).
Minimum 2 years of case management experience.
Experience in acute care/hospital setting (required).
Proficiency with Epic charting.
Strong knowledge of discharge planning, care coordination, utilization review, and community services/resources.
Experience with adult protective services, guardianship, and crisis intervention.
Ability to educate patients and families on plans of care, resources, and discharge processes.
Ability to work with diverse populations and understand cultural and psychosocial considerations.
Strong assessment, documentation, and communication skills.
Ability to float between units and affiliated facilities as needed.
Preferred Skills & Experience:\nTeaching hospital or community hospital experience.\nExperience with:\nAppeals & denials\nPrior authorizations\nHospice and home health coordination\nNursing home admissions and transfers\nKnowledge of Medicare, Medicaid, Social Security, and medical transportation programs\nWorking with patients with communication challenges (speech/hearing)\nComplex medical conditions including CHF, COPD, cancer, stroke, dementia, psychiatric disorders, etc.\nFamiliarity with NASW Code of Ethics and standards of practice.\nExperience in ED, ICU, med-surg, or short-term acute care social work settings.\n
