Position Summary: Health and Wellness Director / DON (RN) As our
Health and Wellness Director , you'll oversee all aspects of resident health and wellness programs while leading and supporting a talented clinical team. Your goal: ensure residents receive the highest quality care with dignity, compassion, and respect.
Key Responsibilities Include: - Lead, mentor, and inspire care and nursing staff in alignment with Charter's mission and values
- Oversee resident health assessments, care plans, and medication management
- Ensure full compliance with Connecticut Department of Public Health assisted living regulations
- Coordinate care services and maintain accurate documentation and reporting
- Partner with the Executive Director and department leaders to deliver excellence in resident outcomes and satisfaction
- Communicate effectively with residents, families, staff, and healthcare partners
- Maintain a culture of teamwork, accountability, and continuous improvement
Qualifications Active RN license in the State of Connecticut (required)
Minimum 2-3 years of nursing leadership experience in senior living, assisted living, or long-term care (preferred)
Strong understanding of
Connecticut DPH and assisted living regulations Excellent leadership, communication, and team-building skills
Compassionate, proactive, and resident-centered approach to care
